Sapp failed to renew his license by December 31, 2017, as required by applicable statute and regulation, and he continued to perform the duties of a physical therapist assistant with an expired/delinquent license. Sapp also failed to complete 24 hours/units of board approved continuing education within the applicable two-year licensing period. |
| Penalty: |
Mr. Sapp's license shall be reprimanded and at his own expense, shall enroll in and successfully complete six hours of on-line board approved course(s) in ethics. Sapp shall submit written proof of completion of these hours to the Board. These hours are in addition to what is otherwise required in order to maintain licensure as a physical therapist assistant and shall be completed within 60 days from entry of the order. Sapp shall be subject to a mandatory audit of his continuing education hours prior to the next renewal of his license. Sapp shall contact the Board prior to submitting a renewal application so that the Board may confirm he has the necessary board approved continuing education hours for renewal. |
