On December 31, 2019 Meade complete a renewal application attesting that she had completed the required 24 hours of Board approved continuing education. Meade was selected for a continuing education audit. Meade's audit letter was returned to the Board as unclaimed. The Board attempted to contact Meade via email and telephone call without success. The Board issued a complaint against Meade for failure to provide confirmation that she completed the necessary Board-approved continuing education hours for the applicable two-year licensing period. Meade then failed to respond to the Board initiated complaint hereby failing to confirm she had completed the necessary Board approved continuing education hours. |
| Penalty: |
After due diligence the Board is unable to locate Meade and hereby suspends her license prior to having a hearing. If after due diligence the Board still cannot locate Meade and have her respond to the matter within 30 days after the effective date of this suspension order, the Board may revoke Meade's license without holding a hearing in accordance with W. Va. Code §30-1-8(e)(2). |
