Violation: |
In March 2019, Ms. Dunn was selected for a continuing education audit for the 2017-2018 reporting period. Ms. Dunn was notified of the continuing education audit selection; however, the audit letter was returned to the Board as unclaimed. The Board office contacted Ms. Dunn's last known employer but was informed that she no longer is employed there. The Board attempted to contact Ms. Dunn via email without success. On March 18, 2020 via certified and regular mail, the Board issued a Board initiated complaint against Ms. Dunn for her failure to confirm she completed the necessary Board-approved continuing education hours for the applicable two-year licensing period and possibly misrepresenting that she had completed such hours when she submitted her renewal application. Both letters were mailed to Ms. Dunn's last known address and both letters were returned. Ms. Dunn has not responded to the Board's complaint.
On July 31, 2020 Ms. Adkins (Dunn) contacted the Board after finding out from her employer that her license had been suspended and claimed that she had completed the necessary continuing education hours and that she could provide documentation of that. Now that Ms. Adkins (Dunn) has made contact with the Board her license has been reinstated, however she must provide proof that she has completed the necessary 24 hours of board approved continuing education within the applicable two year licensing period within 10 business days but August 24, 2020. Failure to do so may result in further disciplinary action being initiated against her license. |
Penalty: |
Based on the foregoing, Ms. Dunn's license to practice physical therapy is hereby suspended and is directed to surrender her license to the Board immediately.
If after due diligence, the Board still cannot locate Ms. Dunn 30 days after the effective date of this suspension, the Board may revoke Ms. Dunn's license without holding a hearing in accordance with W. Va. Code § 30-1-8(e)(2).
On August 3, 2020 Ms. Adkins' (Dunn) license was reinstated pending that she completed the necessary 24 hours of board approved continuing education within the applicable two year licensing period and that provides proof of compliance within 10 business days by August 24, 2020. Failure to do so may result in further disciplinary action being initiated against her license |
